The CCPHP program provides step-by-step, expert assistance to leading physicians as they convert their practices to a concierge (membership) model. The result will be enhanced patient relationships, opportunities for increased practice revenue, and a decreased administrative load. In short, our approach helps physicians bring back the joy of practicing medicine.
Our methodology blends two essential management components – physician practice conversion and practice development.
Practice Conversion includes:
- In-depth practice analysis and detailed business planning, including customized program and pricing development
- Establishment of a joint-venture Membership corporation, including all legal contracts
- Creation of all documentation, Membership Agreements, and regulatory support
- Comprehensive patient and provider messaging, communications, website, social media, and search marketing strategy, as well as turn-key communications development

John Connolly, Ed. D.
John Connolly, Ed. D – the nation’s foremost expert on identifying top physicians, is the President & CEO of Castle Connolly Medical Ltd. publisher of America’s Top Doctors ® and other consumer guides to help people find the best healthcare. He is also Chairman of Castle Connolly Private Health Partners, LLC, a concierge medicine company.
Dr. Connolly served as President of New York Medical College, the nation’s second largest private medical college, for more than ten years. He is a Fellow of the New York Academy of Medicine, a Fellow of the New York Academy of Sciences, and a Director of the Northeast Business Group on Health. He was formerly a member of the President’s Council of the United Hospital Fund, and a member of the Board of Advisors of the Whitehead Institute for Biomedical Research.
Dr. Connolly has served as a trustee of two hospitals and as Chairman of the Board of one. He is extensively involved in healthcare and community activities and has served on a number of voluntary and corporate boards including the Board of the American Lyme Disease Foundation, of which he is a founder and past chairman, and the Culinary Institute of America for over 20 years where he is now Chairman Emeritus. He also served as a Director and Chairman of the Professional Examination Service and Chairman of Alpha Gene Inc. and is presently on the board of the American Swiss Foundation. His current corporate board service includes: Baker and Taylor; Dearborn Risk Management, Air Methods Inc., and the Advisory Board of the Hudson Group. He holds a Bachelor of Science degree from Worcester State College, a Master’s degree from the University of Connecticut, and a Doctor of Education degree in College and University Administration from Teacher’s College, Columbia University. He as been awarded honorary doctorates from Mercy College (LHD), Worcester State University (LHD), and Daemen College.
Chairman of Castle Connolly Medical, Ltd.
John K. Castle, the Chairman of Castle Connolly Medical Ltd., has spent much of the last three decades involved with healthcare institutions and issues. Mr. Castle served as Chairman of the Board of New York Medical College for eleven years, an institution where he served on the Board of Trustees for twenty-two years.
Mr. Castle has been extensively involved in other healthcare and voluntary activities as well. He served for Five years as a commissioner of the Joint Commission formerly known as (JCAHO), the body which accredits most public and private hospitals throughout the United States. Mr. Castle has also served as a trustee of five different hospitals in the metropolitan New York region, including New York Presbyterian Hospital, where he continues to serve.
Mr. Castle has also served as the Chairman of the Columbia Presbyterian Science Advisory Council as Director Emeritus of the United Hospital Fund. He is a Life Member of the MIT Corporation, the governing body of the Massachusetts Institute of Technology.
Mr. Castle’s goal, as is the goal of Dr. John Connolly and all of the Castle Connolly team, is to publish America’s Top Doctors®, America’s Top Doctors® for Cancer, Top Doctors: New York Metro Area, and other materials as well as build websites to help the public identify the very best in healthcare resources.
Mr. Castle received his bachelor’s degree from the Massachusetts Institute of Technology, his MBA as a Baker Scholar with High Distinction from Harvard and two Honorary Doctorate degrees.
Dr. Dean McElwain
President & COO
Dr. McElwain is a seasoned healthcare executive with nearly 30 years of progressive leadership in a variety of healthcare settings. Dr. McElwain has a professional doctorate (DPT) from Simmons College Graduate School for Health Studies, with a specialty in Health Care Administration.
A recognized national thought leader in the area of Concierge Medicine, Dr. McElwain has provided guidance, leadership and innovation to more than 70 providers, as well as regional and national corporate clients over the last 9 years. Renowned organizations, such as the Lown Cardiovascular Institute, have relied on Dr. McElwain’s guidance for effective design and management when evaluating and or transitioning their practices to the subscription-based or concierge membership model.
A lifelong endurance athlete, Dr. McElwain is also a respected leader in the areas of Health Coaching, Wellness and Human Performance. Having completed undergraduate studies in exercise physiology as well as graduate studies in Physical Therapy, Dr. McElwain combines his passion and expertise in the areas of exercise, nutrition and wellness in leading a robust initiative to incorporate wellness into the practices and members he guides.
Dr. McElwain is a member of the American Academy of Private Physicians (AAPP) as well as the Healthcare Financial Managers Association (HFMA) and is an external consultant member of the Medical Group Managers Association (MGMA).
Dan Nauheim is the Chief Financial Officer of Castle Connolly Private Health Partners, LLC. Prior to joining Castle Connolly Private Health Partners LLC, Dan was the Assistant Controller at a venture capital firm focused on sports & entertainment. Dan spent several years working in healthcare finance at prestigious hospitals in the New York City area, such as NYU Langone Medical Center, and Northwell Health. Dan is a CPA, and earned his MBA in Finance from Baruch College.
Vice President and General Counsel
Ashley is an experienced healthcare attorney, with a background in advising healthcare clients on regulatory matters, including federal and state legal compliance and policy issues impacting client businesses. Prior to joining Castle Connolly Private Health Partners, Ashley was an associate in the healthcare group of Manatt, Phelps and Phillips, LLP and the litigation and healthcare regulatory practice groups at Hogan Lovells LLP. Before law school, Ashley worked at a non-profit organization in San Francisco, conducting research on improving quality of care for Medicare beneficiaries.
Ashley earned her Bachelor’s degree, magna cum laude from Brown University and her law degree, magna cum laude from the Benjamin N. Cardozo School of Law. She is admitted to practice law in New York State.

VP, Strategy & Engagement
With over 25 years of experience in marketing and advertising, Sean joined Castle Connolly Private Health Partners, LLC in 2017. Prior to joining CCPHP, he served as the SVP, Chief Marketing Officer of The Trust for Public Land, a leading national nonprofit environmental organization and he oversaw marketing for Best Buy’s $14 billion computing business unit as the Senior Director of Marketing. Prior to Best Buy, Sean was a VP of Marketing at XM Satellite Radio in Washington DC, where he guided a rebranding and marketing campaign that helped draw millions of subscribers to XM and was named one of the best ad campaigns of 2006 (in a USA Today poll). Sean also oversaw corporate partnerships for XM, leading business development and co-marketing with leading companies such as Starbucks, JetBlue, Avis and United. He joined XM from Intel Corporation, where he worked in marketing for just over seven years, ultimately acting as the Worldwide Advertising Director and overseeing the development of all consumer and B@B campaigns, including the world renowned “Blue Man Group” campaign and the worldwide launch of Centrino wireless mobile technology.
Sean got his start in advertising, working for some of the top Agency brands in the 90s, including Grey in New York City and FCB in New York City, Hong Kong and San Francisco. Sean received his bachelor’s degree from Colgate University and he now lives in Westchester, NY.
