Dr. McElwain is a seasoned healthcare executive with over 30 years of progressive leadership in a variety of healthcare settings. Dr. McElwain has a professional doctorate (DPT) from Simmons College Graduate School for Health Studies, with a specialty in healthcare administration, having earned that program’s highest honor of academic distinction.
A recognized national thought leader in the area of concierge medicine, Dr. McElwain has lectured extensively, consulted with leading companies, and provided guidance, leadership, and innovation to hundreds of physicians, as well as regional and national corporate clients, over the last 13 years. Renowned organizations such as the Lown Cardiovascular Institute and leading academic medical institutions have relied on Dr. McElwain for strategic guidance and effective deployment when transitioning their organizations to the subscription-based or concierge membership model.
A lifelong endurance athlete, Dr. McElwain is also a respected leader in the areas of health coaching, wellness, and human performance. Dr. McElwain has completed undergraduate studies in sports medicine/exercise physiology, as well as extensive post-graduate studies in physical therapy and human performance.
Dr. McElwain combines this passion and expertise in leading a robust set of initiatives that share the benefits of the wellness-centered lifestyle with all members of the Castle Connolly Private Health Partners family.

With over 30 years of experience in marketing and advertising, Sean joined Castle Connolly Private Health Partners, LLC in 2017. Prior to joining CCPHP, he served as the EVP, Chief Marketing Officer of The Trust for Public Land, a leading national nonprofit environmental organization and he oversaw marketing for Best Buy’s $14 billion computing business unit as the Senior Director of Marketing. Prior to Best Buy, Sean was a VP of Marketing at XM Satellite Radio in Washington DC, where he guided a rebranding and marketing campaign that helped draw millions of subscribers to XM and was named one of the best ad campaigns of 2006 (in a USA Today poll). Sean also oversaw corporate partnerships for XM, leading business development and co-marketing with leading companies such as Starbucks, JetBlue, Avis and United. He joined XM from Intel Corporation, where he worked in marketing for just over seven years, ultimately acting as the Worldwide Advertising Director and overseeing the development of all consumer and B@B campaigns, including the world renowned “Blue Man Group” campaign and the worldwide launch of Centrino wireless mobile technology.
Sean got his start in advertising, working for some of the top Agency brands in the 90s, including Grey in New York City and FCB in New York City, Hong Kong, London, and San Francisco. Sean received his bachelor’s degree from Colgate University and he now lives in Westchester, NY, with his daughter, twin boys, and his dog Jake.

Andres Jimenez is the Chief Financial Officer for Castle Connolly Private Health Partners, LLC. (CCPHP).
With over 20 years of experience in healthcare, Andres has an array of experience working with private equity, senior creditors, managing financial audits, valuations, Q of E reviews, and has recruited, trained, and developed finance teams in fast pace high growth environments. Prior to joining CCPHP, Andres was Controller at Clinilabs, a CNS Drug Development company, and Controller and Director of Finance at eHealth Solutions, Inc., a healthcare information technology firm, both located in New York. Earlier in his career, Andres was an Analyst and controller in Marketing, Education, and Healthcare organizations like Grey Global Group, Columbia University, and CarePlus.
Andres is a graduate of Baruch College in New York, NY with a Bachelor of Business Administration in Accounting.
Andres lives in Queens, NY with his wife and two daughters and is an avid Crossfitter. In his spare time, he volunteers to train and develop youth in AAU Basketball and Travel Softball.

Andrew Bogen, Esq. has more than 35 years of experience counseling clients in transactional, regulatory and other matters in the healthcare industry.
He is the founder of The Law Office of Andrew Bogen and before that, he served as a Partner, Counsel and Associate at top tier national and boutique law firms, including Arnold & Porter and Arent Fox.
Mr. Bogen was on the Law Review and received his law degree, with honors, from the George Washington University National Law Center. He earned his BBA, with distinction, in marketing and finance from Emory University’s Goizueta Business School. Andrew is very pleased to be a part of the CCPHP Team that is greatly enhancing physician-patient relationships.

Alfredo is the Chief Growth Officer at Castle Connolly Private Health Partners (CCPHP). Prior to joining CCPHP, he was a Principal with ECG Management Consultants, a healthcare-only focused advisory firm.
Alfredo advised on over $3B of board-approved investments, working with leading health systems and medical groups on a range of growth initiatives including joint venture planning, partnership & affiliation development, capital asset planning, and digital health development, among other enterprise-level corporate strategies. Clients included Cleveland Clinic, Intermountain Healthcare, Advocate Aurora Health, Yale New Haven Health System, University of California Irvine Health, and Houston Methodist.
Alfredo received his Bachelor of Science in finance from Lehigh University and his Master of Business Administration from NYU’s Stern School of Business. He was captain of his college tennis team. He enjoys spending his free time pretending he can still play tennis and hanging out with his family and newborn son.

Susan is SVP and National Director of Wellness Services at Castle Connolly Private Health Partners (CCPHP), a concierge medicine organization, where she runs the SENS Solution® Wellness Program focused on four wellness pillars: sleep, exercise, nutrition, and stress management. In 2021 CCPHP launched the SENSe of Wellness podcast, hosted by Susan.
Susan has nearly 30 years’ experience as a registered dietitian nutritionist (RDN) and health and wellness professional. In her past experiences, Susan helped develop and deliver lifestyle change programs focused on stress & weight management and physical activity. Susan is the author of a diet/cookbook, entitled “Cooking with Trader Joe’s: Lighten Up!” and is the creator of and talent for two television cooking series: “Lighten Up with Susan Greeley” and “Simply Healthy”.
As a food, nutrition, and wellness expert, Susan specializes in weight management, chronic disease prevention and management, wellness & lifestyle coaching, integrative nutrition, culinary medicine, and healthy cooking. In addition, Susan has been a consulting nutritionist to the Executive Health Program at Atlantic Health System (Morristown, NJ) for more than 10 years and is a Chef Instructor in the Health
Supportive Culinary Arts program at the Institute of Culinary Education in New York City.
Susan is a member of the National Wellness Institute (NWI) and the Academy of Nutrition and Dietetics (AND), as well as the Dietitians in Integrative & Functional Medicine (DIFM) and Food and Culinary Professionals (FCP) practice groups of the AND. Susan holds a Master of Science Degree in Nutrition Communication from Tufts University in Boston and earned her Bachelor of Science degree in Dietetics from Miami University in Oxford, Ohio.

Salena Chu is Vice President of Membership Services at Castle Connolly Private Health Partners (CCPHP) responsible for member engagement and relationship management with partner physicians. Salena started her career at CCPHP as a Member Advisor, promoted to an Account Executive and then shortly to Director of Membership Services. In her current role, Salena leads a team of member advisors to drive membership growth for the company and maintaining relationships with partner physicians to ensure physician satisfaction as well as member satisfaction.
Prior to joining CCPHP, she has eight years of hospitality experience so customer service is a passion for Salena. She believes that always going above and beyond every customer interaction ensures you are creating a meaningful connection for a positive experience.
Salena is a graduate of Baruch College in New York City with a Bachelor of Business Administration in Marketing. She speaks fluent Cantonese. During her spare time, Salena enjoys spending time with her husband, two young children and karaoke.

Eli Pollack, Esq. is the Vice President of Legal Affairs at Castle Connolly Private Health Partners (CCPHP). Eli is responsible for supporting the company’s General Counsel, as well as collaborating with all team members regarding legal affairs of the company. Areas of expertise and focus include joint venture contract negotiations, new partnership agreements, mergers and acquisitions, as well as ensuring compliance with relevant laws and regulations towards minimizing the company’s exposure to legal risks.
Prior to joining CCPHP, he was legal counsel and chief operating officer at a large independently owned physician network that operates and organizes high-risk preventive managed care for Medicare and Medicare Advantage lives.
Eli has an eclectic and interdisciplinary background of business, operations and law. In his previous role, Eli performed health insurance reimbursement analysis and spearhead all key initiatives for ensuring and improving clinical and financial outcomes for large physician networks, which included patient risk stratification, expense vs. premium funding, cost and utilization review, developing preferred specialist delivery systems, overseeing all chronic and transitional care management teams, improving quality care metrics and managed all risk adjustment coding in order to improve and predict healthcare costs. Furthermore, he also advised on practice acquisitions, employment contracts, vendor agreements, negotiated all preferred provider arrangements, referral systems, waivers and was heavily involved in partnership and joint ventures between various companies in the healthcare industry.
Eli is a certified risk coder and received his Bachelor of Science in Biomedical Engineering with a concentration in Tissue Engineering from North Carolina State University. He was on the Law Review and received his Juris Doctorate, with honors, from Nova Southeastern University – Shepard Broad College of Law. Eli lives in South Florida with his wife and two daughters, enjoys playing golf and spending as much time as he can outside at a pool or beach.

John Connolly, Ed. He is also Chairman of Castle Connolly Private Health Partners, LLC, a concierge medicine company and is a director of TopDoctors.es (Europe and Latin America) and Medebound (China).
Dr. Connolly served as President of New York Medical College, the nation’s second-largest private medical college, for more than ten years. He is a Fellow of the New York Academy of Medicine, a Fellow of the New York Academy of Sciences, and served as a Director of the Northeast Business Group on Health for 25 years. He was formerly a member of the President’s Council of the United Hospital Fund and a member of the Board of Advisors of the Whitehead Institute for Biomedical Research.
Dr. Connolly has served as a trustee of two hospitals and as Chairman of the Board of one. He is extensively involved in healthcare and community activities and has served on a number of voluntary and corporate boards including the Board of the American Lyme Disease Foundation, of which he is a founder and past chairman, and the Culinary Institute of America for over 20 years where he is now Chairman Emeritus. He was also the founder and first chairman of the Dutchess County (NY) Industrial Development Agency. He also served as a Director and Chairman of the Professional Examination Service and Chairman of Alpha Gene Inc. and was also on the board of the American Swiss Foundation. His former corporate board service includes Baker and Taylor; Dearborn Risk Management, Air Methods Inc., and the Advisory Board of the Hudson Group. He holds a Bachelor of Science degree from Worcester State College, a Master’s degree from the University of Connecticut, and a Doctor of Education degree in College and University Administration from Teachers College, Columbia University. He has been awarded honorary doctorates from Mercy College (LHD), Worcester State University (LHD), and Daemen College (LL.D.).

Proper nutrition and a safe environment are strong determinants of a person's health and wellness.
That's why, we've partnered with The Bowery Mission over the past 5 years to provide over 2,500 meals and 415 individuals a full day of compassionate care at the Bowery Mission campus, complete with 3 hot meals, warm showers, clean clothes, hygiene items, and clinical care evaluations to those in need.
Unclean air has been linked to diseases including several types of cancer, asthma, neurological disorders, liver and skin disease, and more. The quality of the air we share can be improved by trees, but the number of trees on our planet decreases year over year due to deforestation.
That’s why, on behalf of our partner physicians, we worked with Evertreen to plant 1,000 trees, absorb 250 tons of CO2 emissions, and create 125 working hours for farmers in Tanzania.
