Meet The Doctors

Dr. Horbar received his medical degree from New York Medical College. He was chief resident at Lenox Hill Hospital while in his internal medicine residency program. Currently, Dr. Horbar is an instructor at the medical school and residency program at Lenox Hill. Dr. Horbar has been honored with accolades such as being cited as “Top Doctors: New York Metro Area” by New York Magazine more than a dozen times and was the first recipient of the Master Clinician Award by the “Department of Medicine”.

Dr. Hochweiss’ passion for medicine started early when she attended The Bronx High School of Science. She received her medical degree from New York Medical College and conducted her internal medicine internship and residency at Lenox Hill Hospital. Following her residency, Dr. Hochweiss began her hematology fellowship at Mount Sinai Hospital and combined her love of travel and medicine by continuing the program at Hammersmith Hospital in London, England. She also completed a fellowship in oncology at Columbia Presbyterian Medical Center. Dr. Hochweiss is boarded in both internal medicine and hematology. Currently, Dr. Hochweiss is a preceptor at SUNY Downstate Medical Center and teaches at Lenox Hill Hospital.

Gary Horbar, MD, has been in practice with his wife, Dr. Sharon Hochweiss, for nearly 25 years. With a focus on patient-centered care, he has developed relationships with his patients through a collaborative and connected approach.

Dr. Horbar received his medical degree from New York Medical College. He was chief resident at Lenox Hill Hospital while in his internal medicine residency program. Currently, Dr. Horbar is an instructor at the medical school and residency program at Lenox Hill.

Sharon Hochweiss, MD, has been in practice with her husband, Dr. Gary Horbar, for nearly 25 years. Developing a collaborative and connected relationship with her patients is central to her holistic and patient-centered approach to care.

In this model, the physicians of Horbar Hochweiss CCPHP have considerably fewer patients than typical primary care doctors. This allows them to have enough time to provide each and every patient with personalized, unhurried care. Members of Horbar Hochweiss CCPHP pay an affordable annual fee to take advantage of a wide array of member benefits and enjoy a more convenient, connected, and compassionate approach to healthcare.

Membership fees are payable to Horbar Hochweiss CCPHP, LLC, a business entity formed by Dr. Horbar, Dr. Hochweiss and CCPHP that is separate from their medical practice. Our physicians provide all professional services through their practice, Horbar Hochweiss CCPHP. Since the membership fees do not pay for any insurance-covered healthcare services, we recommend that our members maintain their health insurance. Contact us for more information.
